Documents Needed to Complete Interview Process. Please Bring Copies of All Requested Documents As We Are Unable to Make Copies during Interview.

• A Valid Georgia RN, LPN, and CNA License
• CPR/First Aid Certification AED or BCLS
• Crisis Prevention and Intervention (CPI) – If Applicable
  Annual Physical
• A Negative TB Test Results or Chest X-Ray
• Driver’s License
• 7 Years Driving Record
• Current Auto Insurance
• Resume
• Copy of Professional Liability Certificate of Insurance – All Nurses (LPN/RN, etc)
• Copy of All Applicable Certifications, Diplomas, and Degrees
• Cogent Background Fingerprints for ALL Applicants for DD Cases
• Police Background Check – Non-DD Applicants (Must Indicate Ridgewood Personal Care Home Inc.)
• Social Security Card
• Voided Check (For ACH Direct Deposit Processing)
• Five years employment verification
• Three References
