Ridgewood PCH Inc. also offers services under the Community Care Services Program (CCSP)—a Medicaid waiver program designed to provide eligible elderly or physically disabled individuals with community-based alternatives to nursing home care.
In partnership with Georgia’s Area Agencies on Aging, our team works with care coordinators and healthcare professionals to develop customized care plans based on each client’s unique needs. CCSP enables clients to receive essential health and personal care services in the comfort of their own homes or licensed personal care environments.
Available CCSP services through Ridgewood may include:
- Personal support services (e.g., bathing, meal prep, housekeeping)
- Structured Family Caregiver support
- Medication management and health monitoring
- Skilled nursing
- Respite care
