Ridgewood PCH Inc. proudly participates in the Service Options Using Resources in a Community Environment (SOURCE) program, offering coordinated, in-home and community-based care for individuals who qualify for full Medicaid and meet the Intermediate Nursing Home Level of Care.
Through SOURCE, we link primary medical care and case management with essential long-term health services to support individuals in remaining safely and comfortably in their homes or community. This program is ideal for frail elderly or physically disabled adults who require comprehensive care but wish to avoid institutional placement.
